Harm Reduction Outreach Program Report (OCHART Section 6)
The Harm Reduction Outreach Program Report is generated using data entered into the NEP/NSP module in NEO. This report documents client interactions, including unique client codes, gender, services provided, supplies distributed, and locations.
Needle Exchange / Syringe Program Activity Report
The Needle Exchange / Syringe Program Activity Report gathers data from the NEP/NSP module in NEO, covering client demographics, needles distributed and returned, as well as services and referrals provided.
Ontario Naloxone Program Quarterly Report
The Ontario Naloxone Program Quarterly Report is generated using data from the Naloxone module. This report collects information on the distribution of injectable and nasal spray naloxone, as well as the number of individuals trained.
CTS Monthly Report
The CTS Report gathers data from the CTS modules in NEO. It includes information on client demographics, supervised consumption visits, overdose events, medical emergency service calls, safety and security, harm reduction services, and wrap-around services. Additionally, the report contains a tab for Federal Reporting, covering Drug Splitting/Sharing, Peer Assistance, and Drug Checking.
Needle Exchange Program and Naloxone Raw Data Report
The NEP and Naloxone Raw Data Report compiles data from the NEP and Naloxone modules. This report enables agencies to view site information, site users, and transactions related to NEP and naloxone.
CTS Medical Emergency Reports
The Medical Emergency Report gathers information on medical emergencies from the CTS module.
