Instructions:
Use the Search page to access the Client’s Profile page.
The Client’s Profile page includes the following client-specific information:
On the top-left side of the screen:
- Centre Number: Unique code associated with your YWHO site.
- Client ID: Unique client ID.
- Client First and Last Name: Client’s first and last name.
On the left side of the screen:
- Visit Lists:
- Registration Form: Includes client information collected at registration.
- Visit number and date: Includes a list of all of the client’s visit number, date, and form information for each visit.

- Form Information: Includes all data that has been collected for a particular form (e.g. Reason for Visit form).
On the bottom-left side of the screen:
- Start Client Data Entry button

Icon | Status | Description |
| New | The form has not been opened |
| In Progress | The form has been started and saved as a draft |
| Validated | The form has been submitted and completed |
