When a client answers the Patient Health Questionnaire (PHQ-9) or the Gain-SS, they may respond to sensitive questions (i.e., suicide risk) in a way that warrants immediate attention of a service provider. In these scenarios, service providers can be alerted directly through the YWHO Data Collection Platform.

The alert will be an email sent to the service provider that the client is tagged to (i.e., Owner). This is done for alerting the service provider who is to provide service to the client that day.

  • Clicking on the link included in the alert email will redirect you to the YWHO data collection platform login page. Once logged in, the platform will automatically redirect you to the form that generated the alert on the Alerts Centre.
  • The link in the alert email notification will expire in 3 months.


Alerts Centre

The Alert Centre will display all the alerts that have been generated for your account. To access the Alert Centre:

  • Hover over the CLIENTS tab on the top-left corner of the screen.
  • Click on the ALERTS CENTRE button.
  • Use the FROM and TO buttons to filter alerts by date.
  • Click on the FILTER DATES button to filter the alerts by the desired dates.

Alert Levels:

  • Level 1 (Green): An alert that has not been cleared – once an alert is opened by a service provider, it is marked as read and cleared.
  • Level 2 (Yellow): An alert that has not been cleared for over 30 minutes is escalated to Level 2.
  • Level 3 (Red): An alert that has not been cleared over 45 minutes is escalated to Level 3.

  • The “No Data to Display” message on the Alert Centre means that your account has not received any alerts.
