Instructions:

Before adding/registering a new client profile in the YWHO data collection platform, use the Search function to confirm that a profile for that client has not already been created. This will help with preventing the duplication of client data.

To add/register a new client profile to the platform:

• Click on the ADD A NEW CLIENT button on the top-left corner of the screen:
  • Clicking on this button will take you to the Form Selection and Data Entry page, or Registration Form.

• Input the following client information into the Form Selection and Data Entry page, noting that some information is automatically generated:
  • Centre Number: Centre number generated automatically.
  • Client ID: Client ID generated automatically.
  • Last Name: The client’s last name.
  • First Name: The client’s first name.
  • Preferred Name: The client’s preferred name. If the client’s preferred name is the same as the client’s first name, you are welcome to leave this field blank or include the first name.
  • Date of Birth: The client’s date of birth. It is important that you confirm with the client that they are providing you with an accurate date, since this information will be needed to pull the client’s profile for recurring visits.
  • Age: The client’s age will be automatically generated after you include the client’s date of birth.
  • This Client is a: For all of the youth clients that you serve, you will want to check off the “Youth” option. If your site is providing family services, you will want to check off the “Caregiver” option for family members.
  • Pre-Existing Client ID (if applicable): This field has been included to give you the option to include other database/platform IDs for this particular client.

• Click on the VALIDATE – LOCK button to finalize the creation of the client’s profile.

• Once the Registration Form has been validated, the Add a New Visit pop-up will open.

• Use the VISIT DATE field to input the date of the client’s visit. The visit date will pre-populate with the current date. You can edit the date using the calendar button beside the field.
