Report Categories:

Harm Reduction Outreach Program Report (OCHART Section 6)

Ministry Report: A01 - The Harm Reduction Outreach Program Report is calculated using the data inputted in the NEP/NSP module in NEO. This report records client interactions (unique client code, gender, services delivered, supplies distributed, location). 

Needle Exchange / Syringe Program Activity ReportProgram 

Ministry Report: A02 - The Needle Exchange / Syringe Program Activity Report collects data from the NEP/NSP module in NEO regarding client demographics, needles distributed and return as well as services and referrals that have been provided.

Ontario Naloxone Program Quarterly Report

The Ontario Naloxone Program Quarterly Report is calculated using data from the Naloxone module. This report collects data that pertains to injectable naloxone distributed, nasal spray naloxone distributed, and individuals trained.

CTS Monthly Report

The CTS Report collects data from the CTS module in NEO. The repots collects data regarding client demographics, supervised consumption visits within the CTS, overdose events, medical emergency service calls, safety and security, harm reduction services, wrap around services.

This report also includes a tab for Federal Reporting as it relates to Drug Splitting/Sharing, Peer Assistance and Drug Checking.
