Instructions:

Pre-Session

After a new visit has been created for the client and before the client is asked to complete any of the forms (i.e., sociodemographic and clinical assessments), there are two forms that must be completed by service providers. Once these two forms are completed, service providers can proceed with collecting the appropriate data from clients for their visit.

  1. Service Request Form (SERVICE_REQUEST): this form is used to document the service that the client is requesting when they connect with a service provider.
  2. Start of Visit Form (START_VISIT) – Add Provider of START_VISIT: this form is used to link the client to the appropriate service provider for the current visit.

To complete the Service Request Form (SERVICE_REQUEST) form:

  • On the client’s profile, click on the SERVICE REQUEST FORM (SERVICE_REQUEST) on the left hand side of the screen under the appropriate visit.
  • To select the type of service that the client is requesting:
    • Click on the EDIT button.
    • Select the desired type of service(s).
  • Once complete, click on the VALIDATE – LOCK button.

To complete the Start of Visit Form (START_VISIT) – Add Provider of Start_Visit form:

  • On the client’s profile, click on the START OF VISIT FORM (START_VISIT) form on the left side of the screen under the appropriate visit.
  • Click on the ADD PROVIDER OF START_VISIT form.
    • Repeat this step if you need to tag more than one service provider to the client.
  • Use the OWNER drop-down menu to select the appropriate service provider based on a) client’s needs and b) available of service providers that day.
  • Once complete, click on the VALIDATE – LOCK button.
Notes:
  • Assigning an owner is a critical step because alerts will be forwarded to the tagged service provider (“Owner”). Tagging a service provider indicates that the youth/caregiver requires services from them that day.
  • You are able to tag as many service providers as needed to be designated “owners” for each visit.

Post-Session

After the session with the client is over, service providers complete the END OF VISIT FORM (END_VISIT) – ADD OCCURRENCE OF END_VISIT form.

To complete the END OF VISIT FORM (END_VISIT) – ADD OCCURRENCE OF END_VISIT form:

  • On the client’s profile, click on the END OF VISIT FORM (END_VISIT) form on the left hand side of the screen under the appropriate visit.
  • Click on the ADD OCCURRENCE OF END_VISIT form.
  • To select the type of service that the client received:
    • Click on the EDIT button.
    • Select the service(s) that the client received.
  • Once complete, click on the VALIDATE – LOCK button.
