This document identifies the data elements captured at each client stage and in common scenarios, based on the OSP Minimum Data Set (V1.12).

  1. Client completes service at assessment NLO and exits OSP

  2. Client completes assessment and triaged to low intensity service provider

    1. Triaged to BounceBack or CAB

    2. Triaged to iCBT

  3. Client begins service at assessment NLO and becomes unreachable

  4. Client begins service at assessment NLO and is put on hold for psychiatric assessment

  5. Client begins/ completes low-intensity service, and is stepped up for high-intensity treatment

  6. Client completes service at assessment NLO and receives booster sessions before exit

  7. Client begins/ completes treatment for primary main problem descriptor, and receives treatment for a secondary main problem descriptor before exit

  8. Client transferred to low-intensity treatment

    1. Completes high-intensity treatment and referred internally to low-intensity service

    2. Begins high-intensity treatment and stepped down to low-intensity service
