Instructions:
The Progress Tracker report is a tool that will allow you to measure the client’s improvements and/or deterioration over a period of time using three of the YWHO clinical assessments.
To generate a Progress Tracker report:
- Hover over the REPORTS tab on the top-left corner of the screen.
- Click on the PROGRESS TRACKER button under the Reports tab.
- On the left side of your screen, under the SELECT A DASHBOARD and the SUMMARY STATISTICS section, click on the PROGRESS TRACKER.
- To select the client to generate a progress tracker for:
- Click on the SELECT button beside the client field.
- In the Select a Client pop-up, select the desired client.
- Use the FORM drop-down section to select the form that you want to generate a progress tracker for.
- Use the START DATE and END DATE sections to choose the timeframe for the progress tracker report or leave these fields blank to generate a progress tracker with all client data.
- Click on the SUBMIT button to generate the progress report.
Notes:
- At this time, you can generate progress tracker for the Kessler Psychological Distress Scale (K10), Self-Rated Health (SRH) tool, Self-Rated Mental Health (SRMH) tool, and SOFAS clinical assessments.
- Leaving the START DATE and END DATE sections blank will generate a progress tracker with all client data.
- You can download a PNG file of the progress tracker report by click on thebutton at the top-right hand side of the Progress Tracker report.
