Lists clients by wait list.

Report Columns

  • Program Name – The agency program name
  • Program ID – The program ID of the agency program
  • Wait List # - The wait list number of the wait list
  • Wait List Status – The wait list status
  • Start Date – The wait list start date
  • Name – Client name
  • Gender – Client’s gender
  • DOB – Client’s date of birth
  • Chart/File # - Client’s chart/file number
  • Home Phone # - Client’s home phone number
  • D/R – Indicate whether the client is DATIS registered or not
  • Last Adm - Client’s latest admission date
  • Caseworker – The caseworker of the client for the wait list
  • Wait List Site No – The wait list site number
  • Date In – The date on which the client is put on a wait list
  • Date Out - The date on which the client is removed from the wait list.
  • No of days in Wait List – Date Out – Date In
  • Status – The client’s wait list status: Active, Received, or Removed.
  • Reason for Removal from Wait List – the reason the client is removed from the wait list.

Parameters

  • Program Start Date Range – specifies the program start date range to report on.
  • Program ID – specifies the program ID of the agency program that the wait list is linked to. Users can enter ‘*’ to include all agency programs.
  • Wait List No – specifies the wait list no to report on. Users can enter ‘*’ to include all wait list no.
  • Wait List Status – specifies the wait list status to be included in the report. The available options are Active, Inactive, or All.
  • Wait List Member Status – specifies the wait list member status to be included in the report. The available options are
  • Active – clients who are active on the wait list,
  • Received – clients who have received services,
  • Removed – clients who have been removed from the wait list, or all.
