Description
Lists the Direct Services (face-to-face and non-face-to-face), Indirect Services, Pre-Entry Services (direct face-to-face, direct non-face-to-face and indirect) and Prevention Awareness Services provided to/scheduled for clients during the selected time period. This report includes the services provided to/scheduled for the client in a group environment.
Report Columns
- Activity Date/Time – Date and time of the client’s appointment
- Staff Name – Service provider with whom the appointment is scheduled
- Site # – Service site at which the scheduled activity is performed
- Group Name – Group name, if the appointment is a group appointment
- Client Name – Client name
- Chart/File # – Client’s chart/file number
- Adm # – Admission number of the admission that the activity is linked to
- Prg # – Program number of the program that the activity is linked to
- Activity – Activity performed at the appointment
- Activity Type – Activity type of the activity
- Client Time – Actual time spent by the client on the activity
Parameters
- Activity Date Range – specifies the activity date range to report on.
- Service Site – specifies the service site to report on.
