Guide for Use:
Opioid substitute examples: methadone, buprenorphine, suboxone. Yes should be selected when at the time of admission the client is being prescribed methadone or another opioid substitute. No should be selected when at the time of admission the client is not being prescribed methadone or an opioid substitute.
