Definition:
A set of values that indicates the reason why the gambling data form was not completed for a client receiving treatment for problem gambling.
Guide for Use:
The reason only becomes required at the time of the admission's discharge and when the form is not completed at the agency providing problem gambling treatment. It is the responsibility of the agency staff to provide this information. For example, the client identified that they had a gambling problem as well as a substance abuse problem. The client received substance abuse treatment services but declined problem gambling treatment services - select Client declined treatment.
