Description

Summarizes the services, including all Direct, Indirect and Pre-Entry Services entered in the Activity Log, provided by a staff during a selected period.
NOTE:
(1) When a program specific report is run, the service a staff provided to a group will be included in the calculation as long as there is one group member whose program number is linked to a program with the specified program ID.
(2) If an activity is for a group of clients that are registered in more than one selected program, the activity is counted once for each of those programs.

Parameters

  • Activity Date Range – specifies the activity date range to report on.
  • Service Site – specifies the service site at which the activity occurred.
  • Staff – specifies the staff to report on.
  • Program ID – specifies the program ID of the programs to report on.

Sub-reports

Summary by Activity Type – Activity Log
Summarizes the services provided at the selected site, by the selected staff, during the selected period, by Activity Type. The services are grouped by activity status, i.e. Occurred and Not Occurred.

Report Columns:

  • Activity Type – The activity type of the services
  • Total # - The number of times the staff has performed activities with the activity type
  • Total % - The percentage of total number of activities that the staff has provided/performed activities with the activity type
  • Total (Minutes) – The total amount of time the staff has spent on performing activities with the activity type. For activities with status “Not Occurred”, the scheduled duration is used in the calculation. For activities with status “Occurred”, the actual time spent is used in the calculation.
  • Total % - The percentage of total time that the staff has spent on activities with the activity type

Summary by Activity
Summarizes the services provided at the selected site, by the selected staff, during the selected period, by Activity. The activities are grouped by activity status, i.e. Occurred and Not Occurred.

Report Columns:

  • Activity – The activity description
  • Total # - The number of times the staff has provided/performed the activity
  • Total % - The percentage of total number of activities that the staff has performed the activity
  • Total (Minutes) – The total amount of time that the staff has spent on performing the activity. For activities with status “Not Occurred”, the scheduled duration is used in the calculation. For activities with status “Occurred”, the actual time spent is used in the calculation.
