Client Information Form
The Client Information Form (CIF) is a standard form required to access any Ontario Health digital services. This form is primarily used to draft and issue the necessary legal agreements to onboarding health service providers.

Legal Agreements
The following are non-executable samples of the legal agreements required to submit the MHA PDS. You cannot sign and return these samples. Executable versions of these legal agreements will be issued to your organization's signing authority after you submit your Client Information Form.
Service Provider Agreement
Services Agreement
This is a general Service Provider Agreement outlining the responsibilities of Ontario Health holds in providing your agency with digital services, such as the protection of privacy, security and confidential information and our compliance with all applicable laws.
All the following agreements are schedules appended to the general Services Agreement
Schedule - Federated Services for Computer Applications
Service providers must sign this agreement to gain access to MHA PDS Provider Reports. This service provides data and reports to MHA PDS submitting service providers.
Your responsibilities are to follow PHIPA and generally protect the privacy and security of client data in administering access to the HSI, e.g. participating in privacy incident reporting, security assessments, etc. as needed
Note: this agreement also covers service provider access to the provincial electronic health record (EHR), so you may notice this terminology within the document; however, EHR access is not currently in scope for MHA PDS 1.0.
Schedule - PKI Services (Application Managed by Client)
Your vendor is responsible for administering Public Key Infrastructure (PKI) services on your behalf. PKI certificates allow your system to securely submit data to Ontario Health.
PDS-compliant vendors have already signed this same agreement with Ontario Health, and are responsible for the services described therein.
Your signing off on this agreements acknowledges that your vendor is taking on these responsibilities on your behalf.
Schedule - Registration and Sponsorship Services
Service providers must sign this agreement to gain access to ONE ID. ONE ID is used to sign into the provider report portal, and many other Ontario Health services.
ONE ID user accounts are created locally by an appointed Local Registration Authority at your agency. These agreement outlines the responsibilities of your agency and this role in administering this service.
