Description:

Lists the Direct Services (face-to-face and non-face-to-face), Indirect Services and Pre-Entry Services (direct face-to-face, direct non-face-to-face and indirect) provided to/scheduled for a client for a period of time. This report includes the services provided to/scheduled for the client in a group environment.
NOTE: The Direct, Indirect and Pre-Entry Services are recorded in the Activity Log.

Report Columns:

Refer to the sub-reports.

Parameters:

  • Activity Date Range – specifies the activity date range to report on.  

  • Chart/File # – specifies the chart/file # of the client to report on.  Note: User can enter multiple chart # to print the report for more than one client.

  • First name – specifies the first name of the client to report on.

  • Last name – specifies the last name of the client to report on.

NOTE: The first name and last name are required if Chart/File # is not available. The first name and last name entered by the users must be exactly the same as the first name and last name of the client. If both the Chart/File # and client name are entered, then the client name would be ignored.

Sub-reports

Direct Services by Client:

Lists the Direct Services provided to/scheduled for a client during selected period.

Report Columns:

  • Name – The client name.

  • Chart/File # - The client’s chart/file number

    Activity Date/Time – The date and time of the client’s Direct Service appointment.

  • Activity – The activity performed/to be performed at the appointment.

  • Group – The group name, if the appointment is a group appointment.

  • Status – The status of the appointment, Occurred or Not Occurred.

  • Comment – The activity comment.

  • Actual Time Spent – The actual time spent on the appointment.

  • Provider – The service provider with whom the appointment is scheduled.

  • Service Site – The service site at which the scheduled activity is performed.

  • Adm – The admission number of the admission that the activity is linked to.

  • Prg – The program number of the program that the activity is linked to.

Indirect Services by Client:

Lists the Indirect Services provided to/scheduled for a client during selected period.

Report Columns:

  • Name – The client name.

  • Chart/File # - The client’s chart/file number.

  • Activity Date/Time – The date and time of the client’s Indirect Service appointment.

  • Activity – The activity performed/to be performed at the appointment.

  • Group – The group name, if the appointment is a group appointment.

  • Status – The status of the appointment, Occurred or Not Occurred.

  • Comment – The activity comment.

  • Actual Time Spent – The actual time spent on the appointment.

  • Provider – The service provider with whom the appointment is scheduled.

  • Service Site – The service site at which the scheduled activity is performed.

  • Activity – The activity performed/to be performed at the appointment.

  • Group – The group name, if the appointment is a group appointment.

  • Comment – The activity comment.

  • Actual Time Spent – The actual time spent on the appointment.

  • Provider – The service provider with whom the appointment is scheduled.

  • Service Site – The service site at which the scheduled activity is performed.

  • Note 2 – The note 2 associated with the appointment.
