When to add a referral

Catalyst has the capability of collecting referral information at two different stages during a client''s contact with an agency; referrals added prior to admission (entry level) to an agency, and referrals added during treatment at an agency and prior to the admission''s discharge.

For a client receiving Entry (pre-admission) services add the referral on the Client Information screen. For a client admitted to the agency add the referral on the Admission Information screen.

Instructions for making non-electronic referrals

1. Click the Referral button either from the bottom of the Client Information screen (for a client receiving Entry services) or the Admission Information screen for pending or admitted clients. The Outgoing Referral List screen will appear on screen.

  •  Adding referrals from the Client Information screen is only possible if there is no information added and saved to the Admission Information screen.Once fields are completed on the Admission Information screen referrals can no longer be added by way of the Client Information screen. (Closed Admissions excepted)
  • You can only add a referral from the Admission Information screen once all required fields are completed. Exception: if the client type is one of the family member types or Non-MoHLTC and A - Client or Family Member, a referral can be added once the save criteria is completed.

2. On the Outgoing Referral List screen, click the Add Referral button at the bottom of the screen. The Outgoing Referral Information screen appears on screen.






3. There are three types of referrals, Referral to Participating Agency, Referral to Non Participating Agency and Internal Referral.
  • The Internal Referral option is only available from the Admission Information screen and does not count as a referral in the discharge criteria. If you do not add an external referral (Referral to Participating Agency or Referral to Non Participating Agency) for your client prior to discharge, you must check off a checkbox on the Discharge Information screen indicating this; you will not be able to save the discharge without doing so.
  •  You can only add one referral at a time.
4. To add a referral, click the small circle (radio button) in the section header of the referral type you wish to add.


5. Fill in the required fields on the screen, including the Referred On date field in the ''Other Referral Information'' section.

6. To save your referral, click the Save button on the bottom of the screen.This will take you back to the Outgoing Referral List screen.
7. To add another referral, repeat steps 2 to 6.

Accessing & editing saved referral records

Referrals records are listed on one of the two Outgoing Referral List screens.

1. To view those referrals made prior to admission; click the Referral button at the bottom of the Client Information screen. All referrals added from the Client Information screen prior to admission will be listed on the Outgoing Referral List screen that is reached through the Client Information screen.

To view those referrals made and added during an admission, click the Referral button at the bottom of the Admission Information screen.
  •  If your client has more than one admission at your agency, there will be multiple Admission Information screens associated with his/her record and you will only have access to the referrals added during an admission from that Admission Information screen.
  •  Many of the fields in saved referral records are not editable.



  • Each referral is assigned a number. This number is permanent; if you delete a referral, you also delete that number. For example, if you have two referrals (#1 and #2) and you delete the second (#2) and then create a third referral, it will be assigned the number 3 and your list of referrals will be missing a 2.
