Summarizes the responses to the questions in the Supplementary Form. NOTE: Forms that have ‘Null' or blank have been either deleted or not filled out yet.
Report Columns
- Question - Lists the questions in the supplementary form and their possible responses.
- Total – The total number of responses.
- Form 1 – number of responses in Form 1
- Form 2 – number of responses in Form 2
- Form 3 – number of responses in Form 3
- Form 4 – number of responses in Form 4
- Form 5 – number of responses in Form 5
- Form 6 – number of responses in Form 6
- Form 7 – number of responses in Form 7
Parameters
- Form Date Range – specifies the date range when the form was created.
- Program – specifies the program the form is linked to. This is one of the questions in the Supplementary Form.
