Description

Lists the entry services provided by service providers.

Report Columns

  • Name – Client’s name
  • Chart/File # - Client’s chart/file number
  • Site No – The site number on the Client Information screen
  • Date – Entry service Date
  • Client Time – The total amount of time of entry services received by the client.  Note 2 – The note 2 value associated with the entry service. Prior to April 1st, 2009, Entry service groups can include both PG and SA clients. These groups will have “Mixed client” as Note 2.
  • Name – Name of the entry service provider
  • Main – Indicate whether the entry service provider is the main entry service provider or not.  Note: In the following columns, the report displays the admission information of the first open/closed admission, if any, after the entry service date. If there is no open/closed admission after the entry service date, then it displays the first pending/incomplete admission after the entry service date.
  • # - Admission number
  • Date – Admission date
  • Status – Admission status
  • Primary worker – the primary worker of the admission
  • Discharge Date – the discharge date of the admission

Parameters

  • Entry Service Date Range – specifies entry service date range to report on.
  • Site No (Client Info) – specifies the client info site to report on. Users can select All Sites to run the report for all sites.
  • Entry Service – specifies the entry service to report on. Users can select All to report on all entry services.
  • Admission Status – users can select one of these values:

    Admission Status
    Not Admitted after the entry servicesInclude the entry service in the report if the client has NOT been admitted on or after the entry service date.
    Note: Pending and incomplete admissions are considered as “non-admissions”.
    Not Admitted within N days of the entry service dateInclude the entry service in the report if the client has NOT been admitted within the specified # of days of the entry service date.
    Note: Pending and incomplete admissions are considered as “non-admissions”.
    Admitted after the entry servicesInclude the entry service in the report if the client has been admitted on or after the entry service date.
    Admitted within N days of the entry service dateInclude the entry service in the report if the client has been admitted within the specified # of days of the entry service date.
    Note: “Admitted” means that the client has an open or closed admission, excluding pending and incomplete admissions.
    AllInclude all entry services whether they have resulted in an admission or not.
  • Entry Service Provider – specifies the entry service provider to report on. Users can select “Agency Level” to run the report at the agency level or select “All Staff” to run the report on all staff.
  • Anonymous – users can select one of these values:

    Anonymous Value
    IncludeInclude entry services provided to anonymous clients and anonymous groups.
    ExcludeExclude entry services provided to anonymous clients and anonymous groups.
    Anonymous OnlyReport on entry services provided to anonymous clients and anonymous groups only.
